What is a Form I-693 immigration medical examination?

Form I-693, Report of Immigration Medical Examination and Vaccination Record, is the document USCIS requires from most green card adjustment of status applicants to confirm there is no medical condition that would make someone inadmissible on health grounds.

It must be completed and signed by a civil surgeon USCIS has specifically designated. Lenox Medical Clinic handles the medical exam only; we do not provide legal or filing advice.

Who needs an immigration medical exam?

Most people applying to adjust status to lawful permanent resident, or green card, need a completed Form I-693. Requirements can differ depending on the immigration benefit being sought.

Applicants who are unsure whether they need the exam, or when to schedule it relative to filing, should confirm with USCIS or their attorney before booking.

What to expect during your immigration physical

The medical exam is usually completed over two visits because the tuberculosis blood test is processed by an outside lab.

Your exam may include:

  • A physical examination performed by Dr. Challa.
  • A review of your medical history.
  • Required laboratory testing, drawn in our office.
  • A tuberculosis blood test (IGRA), sent to an outside laboratory.
  • A review of your vaccination records against what is needed to complete the form.
  • Completion of Form I-693, sealed in an envelope for submission with your immigration paperwork.

What to bring

  • Unexpired photo ID, such as a passport, driver's license, or state ID.
  • All available vaccine records, including childhood records and records from another country, translated into English if needed.
  • Pharmacy vaccine printouts, if any vaccines were given at a pharmacy.
  • Prior TB test documentation, if you have written proof of a previous positive tuberculosis blood test.

$550 for adults 18 and older, $450 for children under 18. This includes the exam, required labs, Form I-693 completion, and the sealed envelope. Vaccines are priced separately based on your records and age.

Before you book

What to know before your immigration physical

We keep the process clear, self-pay, and easy to follow. Bring the items below so we can keep your visit moving and reduce avoidable delays.

Plan on two visits

Plan on two visits, about a week apart. The tuberculosis blood test goes to an outside laboratory and usually takes about a week. The form cannot be signed until the result comes back. We call as soon as it does.

Bring photo ID

Bring photo ID. Passport, driver’s license, or state ID, not expired. We cannot see the applicant without it.

Vaccine records

Bring every vaccine record available, including childhood records and records from another country. Vaccine doses must be on a written record showing the vaccine name and the date it was given. Records in another language need an English translation.

Pharmacy records

If vaccines are filled at a pharmacy, bring the record back. Dr. Challa has to enter each dose on the form. A dose we cannot see is a dose we cannot record.

Chest x-ray

Chest x-ray is not included. It is usually needed when the tuberculosis test is positive, and it is sometimes needed for other medical reasons that Dr. Challa will explain. We send the order to a nearby imaging center, and they bill the applicant directly.

One dose is usually enough

One dose is usually enough for the form. For vaccines given in a series, the first dose is normally enough for us to complete and seal the form. An applicant already partway through a series receives the next dose that is due. The remaining doses are still recommended for the applicant's health.

Children and pregnancy

Applicants under 18 may need more vaccines than those listed here, depending on age and health history. Dr. Challa reviews each child's needs case by case, which can increase the out-of-pocket cost.

Applicants who are 18 may still need one or more childhood vaccines. Dr. Challa reviews these case by case after seeing the records and gives the price at the visit.

If the applicant is pregnant, please tell us when booking. Pregnancy can affect which parts of the examination apply. Dr. Challa reviews all of the requirements individually and explains what they mean for the form.

Self-pay and lab rules

All immigration physicals are self-pay. We do not bill insurance for the examination or the labs, and we do not accept HSA or FSA. Blood work and urine testing for this examination must be collected in our office; testing ordered elsewhere or done before the examination is not accepted. One exception: an applicant who has written documentation of a previous positive tuberculosis blood test should bring it, and Dr. Challa will review whether the test needs to be repeated.
